Welcome to the 2009-10 academic year! We look forward to another great year of Triton Athletics. Please read the following information to ensure you are aware of NCAA requirements.

Graduation Rate and Academic Success Rate
By NCAA rules and regulations we are to provide information to incoming student-athletes regarding our Graduation Rates and our Academic Success Rate. NCAA Initial Eligibility Standards
In order to be eligible for practice and competition at a Division II institution, incoming freshmen student-athletes must meet the NCAA’s initial eligibility standards. These standards consist of a combination of a core course pattern, GPA and SAT/ACT scores. Please click here for further information on NCAA initial eligibility requirements.

Drug Testing Information
The NCAA randomly selects institutions and student-athletes for drug testing throughout the academic year, including the summer. All student-athletes are subject to drug testing before, during or after their competitive seasons. Student-athletes need to be aware of the NCAA Banned Drugs list and should check with the Athletic Trainers for any questions. Click here for the 2009-10 NCAA Banned Drugs list. Student-athletes who test positive are subject to at least a one-year suspension and loss of eligibility.

Many nutritional/dietary supplements contain NCAA banned substances. In addition, the U.S. Food and Drug Administration (FDA) does not strictly regulate the supplement industry; therefore the purity and safety of nutritional/dietary supplements cannot be guaranteed. Impure supplements may lead to a positive drug test. The UC San Diego Athletics Department does not condone the use of nutritional/dietary supplements and the use of such supplements is at the student-athlete’s own risk. Student-athletes should contact the Head Athletic Trainer Kevin Messey (kmessey@ucsd.edu) or Head Strength and Conditioning Coach Rob Mamula (power@ucsd.edu) for further information.

Further information may also be found through the Center for Drug Free Sport, which sponsors a confidential Resource Exchange Center (REC) for student-athletes and athletics staff with questions about nutritional supplements The REC may be contacted toll free at (877) 202-0769 or at the following link. Student-Athlete Online Forms
UCSD ON-LINE FORMS INSTRUCTIONS:

Please read this thoroughly before completing the on-line forms.  Each student-athlete must use the link below to complete their on-line forms.  Please do not use any old links to access the system because your information will be lost.  All student-athletes have to complete this registration process in order to participate in any sport.  In order to access the forms please read the instructions below and then access the on-line forms through the highlighted link below.

SIGN UP TIME PERIODS: this means you must complete your on-line forms within this period of time, only.

  • July 1st – July 30th:  M/W Cross Country, M/W Soccer, Women’s Volleyball.
  • July 13th – August 12th: Mens Water Polo.
  • August 1st – September 1st:  M/W Swimming and Diving, New M/W Basketball.
  • August 10th - September 10th :  Women’s Crew Returners.
  • August 15th - September 15th :  M/W Tennis Returners, Men’s Crew Returners.
  • August 17th - September 17th:  Mens Crew Returners, M/W Basketball, M/W Tennis, Softball, Baseball, Mens Volleyball, Mens Golf, M/W Fencing.
  • August 23rd - September 23rd:  Women’s Water Polo, M/W Track & Field.
  • August 23rd – October 2nd :  M/W Crew Novices.


After you have completed your on-line forms your information will be reviewed by the UCSD Sports Medicine staff and if further information is needed they will contact you.  Please be sure to answer all questions thoroughly and honestly to ensure that no issues will arise with your review.  Incomplete answers will delay your participation.

In order to access the online forms as a student-athlete, you will need to enter the authorization code at “Step 1” of your registration  process.  Remember that this code is case sensitive.  The Authorization Code is: athleteauth4

IMPORTANT INFORMATION Everyone Must Read This:

How do I know if I’m a new athlete?
  A new athlete is someone who was not on an official intercollegiate athletics team roster at UCSD during the last year, Fall 2008 – Spring 2009 seasons.  If you tried out for a team during the Fall 2008 – Spring 2009 seasons and were cut, then you are a new athlete and must create a new account and fill out the forms again.

How do I know if I’m a returning athlete?  You are a returning athlete if you were on an official intercollegiate athletics team roster at UCSD during the last year, Fall 2008 – Spring 2009 seasons. 

How do I know if I need to complete a Student Health Insurance Plan waiver (SHIP)?  To learn about the SHIP plan click here.  The SHIP waiver refers to the “Health Fee Waiver” found on TritonLink.  You can find Health Fee Waiver by clicking here.  At this page, on the left side, is a link called “Health Fee Waiver”.  This is where you can waive the Student Health Insurance Plan.  Only those who qualify can waive this insurance all others must not waive it and keep it as their primary medical insurance coverage.

How do I know if I need to complete a Health Insurance Agreement?  Only these teams need to complete this agreement:  Fall Sports Only!!

  • Men’s and Women’s Soccer
  • Women’s Volleyball
  • Men’s and Women’s Cross-Country
  • Men’s Water Polo
  • Men’s and Women’s Swimming and Diving
    • You (the student-athlete) and your parent/guardians must read, fill-out, and sign this form together.  You must then turn it in to the Athletic Trainer at your First Team Meeting.  You will find this form as you complete the UCSD Online Forms or you can download it here.

This online participation forms program is only compatible with INTERNET EXPLORER.  If you use a different web browser you will experience technical difficulties.
